Fraud, Waste, and Abuse Complaint Form

If you suspect fraud, waste, or abuse in a school district, please contact the local school district office. You may also report suspected school district fraud, waste and abuse to the Office of the State Auditor using their contact form at www.osa.ms.gov/fraud/.

The complaint form below is to be used by Mississippi Department of Education (MDE) employees to report suspected fraud, waste, and abuse at the Mississippi Department of Education. MDE employees may also report suspected fraud, waste and abuse directly to the Office of the State Auditor using their contact form at www.osa.ms.gov/fraud/.

The information reported may be shared with other government agencies that become involved during the course of any related investigation. Please complete as much of the information below as possible.
